Affidavit - Little League Div 1 only

Little League Tournament Preliminary Roster Form

All teams in all Divisions of play are required to submit a Preliminary Team Roster (PTR) in order to nominate their team. Roster Forms are not complete without at least 12 players, two coaches, an accredited Scorer and an accredited Umpire.

Waiver Application Form II(a) or II(d)

Only mandatory for Div 1 Teams that feature a player who resides outside the League Boundary.
